To make an appointment with Dana, please submit the form below.  She will respond within 24 hours, Monday-Friday with times available for a complimentary 30 minute initial Zoom meeting. 

If she can't help you, she will refer you to someone that can. If we decide that we are a match, she will email you her contract, parent questionnaire, and billing info. Once you have returned your forms, we will set up our initial 1 hour consultation meeting.
